Cementless Ttal Hip Arthroplasty


Abstract
The authors analyzed 40 cases of porous coated cementless total hip arthroplasty of Harris-Galante type from 1985 to 1990. The average age was 46.7 years(range; twenty-three to seventy-eight). There were thirty males(34 cases) and five
females(6 cases). The preoperative diagnosis was avascular necrosis of femoral head in 24 cases, sequelae of femoral neck fracture in 6 cases, sequelae of septic arthritis of hip in 4 cases, ankylosing spondysitis in 3 cases and others in 3
cases.
The averge follow up was 49.2 months(36-96 months).
@ES The results obtained were as follows:
@EN 1. The relationship between the mode of fixation and the stability were optimal stability in 32 cases and suboptimal stabiliy in 3 cases among 35 cases of press fit fixation, and optimal stability in 2 cases and definite instability
in 1
case among 5 cases of non-press fit fixation.
2. The average preoperative Harris hip[ score were 51.3 points which improved to 91.7 points at final follw up. Clincal results were excellent in 32 cases, good in 6 cases, and fair in 2 cases
3. The average Harris hip scores according to fixation category of the femoral stem were 93.4 points in optimal stability, 82.6 points in suboptimal stability, and 82 points in definite instability.
4. The stress shielding were observed in 38 cases(95%) which were lst degree in 16 cases, 2nd degree in 17 cases, 3rd degree in 2 cases, and 4th degree in 3 cases by Engh classification.
5. The average Harris hip scores according to stress shielding degree were 93.8 points in lst degree, 93.6 points in 2nd degree, 80 points in 3 rd degree, and 81.7 points in 4th degree.
6. Radiologic features around femoral stem were rounding or resorption of proximal medial femoral neck in 33 cases, radiolucent line in 12 cases, radiopaque line in 12 cases, endosteal sclerosis of femoral stem tip in 5 cases,
endostealhypertrophy in
10 cases,cortical hypertrophy in 3 cases. vertical subsidence in 2 cases, focal osteolysis in 3 cases, and diffuse osteolysis in 2 cases.
